Certified Medical Assistant - Indian Hills Clinic (Full-Time)

LOCATION: Family Health Care of Siouxland – Indian Hills Clinic

POSITION: Certified Medical Assistant

GENERAL SUMMARY OF DUTIES: Provides professional team-based care to patients.  Assists in the continuum of care and in educating patients on their patient-centered medical home.

SUPERVISOR: Clinic Manager

ESSENTIAL JOB DUTIES: (This list may not include all of the duties assigned.)

  1. Performs general clinical care to patients. Screens patients for appropriate information.
  2. Utilize knowledge and skills in the safe implementation of basic preventative and therapeutic nursing care of assigned patient/family as evidenced by documentation and observation of positive patient care outcomes.
  3. Assists with the preparation of equipment and aids physician during treatment, examination, and testing of patients and documents accurately in the patient’s chart.
  4. Responds to and refers incoming telephone calls.  Instructs patient and family regarding medications and treatment instructions, within scope of practice.
  5. Responsible for preparation of exam rooms including re-stocking of treatment areas.
  6. Work collaboratively with other members of the health care team in coordination of the patient’s care as evidenced by other department feedback.
  7. Performs related work as required.

Customer Relations Representative - South Sioux City Clinic (Full-Time)

LOCATION: Family Health Care of Siouxland – South Sioux City Clinic

POSITION: Customer Relations Representative

GENERAL SUMMARY OF DUTIES: The customer relations representative is responsible for providing effective customer service to all customers, including but not limited to, checking patients in and out; in depth knowledge of company programs.

SUPERVISOR: Clinic Manager or Business Office Manager

ESSENTIAL JOB DUTIES: (This list may not include all of the duties assigned.)

1. Greets patients and visitors in a prompt, courteous, and helpful manner.
2. Checks in patients, verifies and updates necessary information. Assists patients with ambulatory difficulties.
3. Answers telephone, screens calls, takes messages, makes appointments and provides information as needed.
4. Checks patients out, making follow up appointments as needed, collecting payments from patients processing them as directed, reconciling money received and cash drawer.
5. Maintains work area and lobby in neat and orderly manner. 6. Performs related work as required.

Licensed Independent Social Worker - Indian Hills Clinic (Full-Time) 32524

POSITION: Licensed Independent Social Worker

GENERAL SUMMARY OF DUTIES:
Provides professional team-based care to patients and assists in the continuum of care and assists in educating patients on their patient centered medical home

SUPERVISOR: Clinic Manager

QUALIFICATIONS:

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental strength and problems related to their diagnosis, illness, treatment and/or life situation
• Formulate, develop and implement a comprehensive psychosocial treatment plan utilizing appropriate clinical social work treatments and interventions
• Assist with billing for services performed and completing appropriate documentation in the patient’s medical record
• Provide quality customer service internally and externally

Medical Lab Assistant - Urgent Care (On-Call) 22324

LOCATION: Family Health Care of Siouxland – Urgent Care (On-Call)

POSITION: Medical Lab Assistant
*required to work 4 shifts per month
*all shifts are scheduled per employee’s availability

GENERAL SUMMARY OF DUTIES:
Performs technical laboratory testing in the clinic laboratory; or
Performs a variety of support services in the clinic laboratory, provides phlebotomy services for all clients and performs limited routine laboratory testing in the clinical laboratory.

SUPERVISOR: Urgent Care Manager

QUALIFICATIONS:

  • Knowledge of laboratory techniques/equipment to perform routine blood and urine tests; and perform quality control checks.
  • Knowledge of medical terminology to communicate with medical staff regarding laboratory procedures and results.
  • Skill in labeling and maintaining all materials for collection.
  • Skill in preparing written reports concerning the results of laboratory tests and input data into computer system.
  • Skill in making mathematical computations utilizing standard formulae.
  • Skill in establishing and maintaining effective working relationships with patients, medical staff, and the public.
  • Ability to read and understand oral and written instructions and follow written protocols.
  • Ability to work independently and to communicate clearly, both verbally and written.
  • Computer data entry experience required.
  • Graduation from high school or GED
  • One year of medical laboratory/phlebotomy experience/certification preferred

Patient Account Representative - Central Billing Office (Full-Time)

LOCATION: Family Health Care of Siouxland – Central Billing Office 

POSITION: Patient Account Representative

GENERAL SUMMARY OF DUTIES:
Monitor accounts receivable for assigned payers. Process incoming checks and prepare deposit. Arrange payment plans for patients and
offer financial assistance when applicable.

SUPERVISOR: Billing Office Manager

QUALIFICATIONS:

ESSENTIAL JOB DUTIES: (This list may not include all of the duties assigned.)
1. Generates various accounts receivable and pending reports, investigates billing problems for resolution.
2. Inputs patient’s data base information which includes insurance and demographic information.
3. Works with patients in securing prepayment sources or financial agreements.
4. Participates with other staff to follow up on accounts until no balance or turned over for collection.
5. Assists with coding and error resolution.
6. Assists with answering the telephone and providing information as requested.
7. Assists with completion of FMLA and disability forms
9. Performs related work as required

RN Certified Diabetic Educator - South Sioux City Clinic (Full-Time) 41824

LOCATION: Family Health Care of Siouxland – South Sioux City Clinic

POSITION: RN Certified Diabetic Educator– Full-Time (Minimum of 35 hours a week)

GENERAL SUMMARY OF DUTIES: Educates patients on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing. Collects patient data to monitor health status and proactively manage the patient’s condition.

SUPERVISOR: CEO and Clinic Manager

QUALIFICATIONS:

  • Minimum 2 years as an RN, 1,000 hours providing diabetes care and education earned within 5 years of the application date.
  • Continuing Education Hours of 15 CEU’s related to diabetes before taking certified exam.
  • Must take and pass CDCES exam within 6 months of hire date.
  • Ability to provide quality customer service
  • Plan, develop, and implement individual and group diabetes education for adults, pediatrics, and their families and caregivers.
  • Instructs patients in the Standards of Care for diabetes.
  • Develop collaborative relationships with other departments, services, and community health care agencies to facilitate and support quality diabetes care.
  • Evaluate the diabetes and health education program on a continuing basis; assesses teachingmethods and education material for both staff and families of patients.

Ultrasound Tech - Imaging Center (On-Call) 41824

LOCATION: Family Health Care of Siouxland – Imaging Center

POSITION: Ultrasound TechOn-Call (less than 24 hours per week)

SUPERVISOR: Imaging Center Supervisor

QUALIFICATIONS:

  • Must be credentialed through the American Registry of Diagnostic Medical Sonographers (ARDMS) with Ob, Abdomen (RDMS), and Vascular (RVT); American Registry of Radiologic Technologists (ARRT) Sonography RT(S) and Vascular RT(VS); Cardiovascular Credentialing International (CCI) Vascular (RVS).
  • Perform a variety of complex specialized tasks utilizing ultrasonic high frequency sound waves to detect abnormalities including masses, abscesses, and stones.
  • Perform ultrasound procedures, Doppler vascular examinations, abdominal studies and color-flow ultrasound, Ob-Gyn procedures as required.
  • Utilize accessory equipment such as selective transducers, multi-format cameras, printers, and PACS workstations.
  • Prepare patients for procedures, explain procedures to patients. Report changes in patient condition to medical staff.
  • Provide expertise in ultrasound technology to physicians and staff to include handling instruments, operating and monitoring equipment and calling out equipment readings.
  • Collaborates in quality control of equipment and filming and obtains service as required. Establish quality control standards in ultrasound and monitor standards; ensure compliance and documentation in ultrasound for the company.
  • Proficient in the performance of ultrasound procedures. Provides direction and instruction to existing personnel and training to new sonographers.
  • Ensures correct charging of ultrasound procedures and supplies on a daily basis.
